At 4 Branches Chinese Medicine Center it is our intention to provide a healing space for each and every person who makes a decision to work with us. To do this with highest integrity and an open heart, we have office policies that are designed to support and maintain a relationship of mutual accountability between us, as practitioners, and our clients.
Preparing for Appointments
Please complete the medical history form online 24 hours before your scheduled appointment. For all in person and virtual consultations, please complete an initial evaluation form before your first appointment. If you have scheduled a virtual consultation please text (336-739-0317), email (4branchescmc@gmail.com), or use our contact form to submit 2-3 tongue pictures the morning of or night before your appointment specifying your name and appointment time. Please click here for more details about taking and submitting a tongue picture. Please note that this form is a secure form hosted on a separate site that is maintained a HIPAA compliant.
Completing this form prior to your visit gives us the opportunity to maximize the time we spend together in our initial meeting.
Rescheduling and Cancelling of Appointments
We ask that you give a minimum of 24 hours notice if you need to reschedule or cancel any appointment you have scheduled with 4 Branches Chinese Medicine Center. There will be a $75 fee for cancelling and rescheduling appointments with less than 24 hours notice. If you schedule an initial appointment and cancel with less than 24 hours notice, to reschedule, we will charge your credit card for the visit at the time you reschedule your appointment.
Herbal Formulations
All herbal formulations are customized in design and preparation for each individual. Once a formula has been prepared payment is due and is nonrefundable.
Payment
Payment is due in full at the time of your office visit or immediately following a virtual appointment (invoice sent via a secure link).

• PayPal (invoicing available by request)
If you prefer to use a direct, fee-free payment method, feel free to ask about available options.
A $35 service fee will be charged for any returned checks. In such cases, payment must be made in full before your next visit. Any alternate payment arrangements must be made in advance.
