At 4 Branches Chinese Medicine Center it is our intention to provide a healing space for each and every person who makes a decision to work with us. To do this with highest integrity and an open heart, we have office policies that are designed to support and maintain a relationship of mutual accountability between us, as practitioners, and our clients.
Preparing for Appointments
Please complete the medical history form online 24 hours before your scheduled appointment. For all private acupuncture, herbal medicine/Chinese food therapy, or medical qi gong, please complete an initial evaluation form before your first appointment. Please note that this form is a secure form maintained by a separate site.
All who are coming in for a first time community acupuncture clinic visit should complete and submit a a community clinic evaluation form – 24 hours before your scheduled appointment using the secure form that is maintained by a separate website.
Completing these forms prior to your visit gives us the opportunity to maximize the time we spend together in our initial meeting.
Rescheduling and Cancelling of Appointments
We ask that you give a minimum of 24 hours notice if you need to reschedule or cancel any appointment you have scheduled with 4 Branches Chinese Medicine Center. There will be a $45 fee for canceling or rescheduling a Community Acupuncture with less than 24 hours notice and a $75 fee for cancelling and rescheduling private sessions with less than 24 hours notice. If you schedule an initial appointment and cancel with less than 24 hours notice, to rescheulde, we will charge your credit card for the visit at the time you make your appointment.
All herbal formulations are customized in design and preparation for each individual. Once a formula has been prepared payment is due and is nonrefundable.
Payment is due in full at the time of your office visit. We accept cash, check, and all major credit cards (Visa, Mastercard, Discover, or American Express). A $35.00 service charge will be automatically charged for any returned checks. You can also pay using Pay Pal. If you want to use Pay Pal, we will send you a invoice that you can pay at the time of your visit.
In the case of returned checks, payment must be made in full before the next office visit. Any other payment arrangements must be determined in advance.
